Patient Appointment Information

What to bring to your first appointment

For your first appointment, please arrive 30 minutes early to complete paperwork. Please bring all available medical records and information with you or confirm that these documents are already on file with University of Colorado Gynecologic Oncologists.

If you have already been diagnosed with gynecologic cancer, please bring all lab and pathology reports, as well as slides, scans, tumor specimens, consultation notes, surgical reports, and other applicable diagnosis information (your previous doctor will be able to provide these to you upon request).

Depending on the type of procedure or appointment you are having, you may be asked to bring additional information. When scheduling your appointment, you will be informed of the exact documents and information you need to bring.

What to bring to your second opinion diagnosis appointment

Second opinions are based on our review of all available information, including our comprehensive review of all previous scans and test. It is important to bring all documentation with you, including all lab and pathology reports, slides, scans, tumor specimens, consultation notes, surgical reports, and other applicable diagnosis information (your doctor will be able to provide all applicable information upon your request).
